Health Care for the Homeless

Health Care for the Homeless (HCH), in partnership with the Harford County Health Department, provides a comprehensive system of health-related services to reduce the incidence and burdens of homelessness in Harford County. HCH offers pediatric and adult primary medical care; in addition to social work and case management services,  at 1 N. Main St., in Bel Air, MD. HCH serves all persons who currently, or have in the past year, lack a fixed, regular, and/or adequate residence.

Medical Services

HCH’s primary care medical clinic is provided every Monday 9am-4pm at 1 N. Main St in Bel Air for children and adults. Please call 410-638-3060 to schedule a medical appointment. Medical services include:

  • Medical history and physicals
  • Primary medical treatment
  • GYN services
  • Vaccines
  • Routine laboratory testing
  • Specialty medical referrals
  • Health education and disease prevention
  • Follow-up medical appointments

Case Management & Social Work Services

HCH’s case management services are available Tuesday – Friday 9am-4pm at 1 N. Main St in Bel Air. Please call 410-638-3060 to schedule an appointment. Case management services include:

  • Biopsychosocial assessments
  • Medical and primary care referrals
  • Information and referrals for food, shelter, entitlements, emergency assistance and shelter
  • Crisis intervention
  • Referrals for substance abuse and mental health treatment/services
  • Prescription and medical supply assistance
  • Health insurance application assistance
  • Information and referral to substance abuse and mental health treatment
  • Individual HIV risk reduction counseling and testing
