HOME AND COMMUNITY-
BASED OPTIONS WAIVER
The Home and Community-Based Options
Waiver (HCBOW), often referred to as the
Community Options (CO) Waiver, is a
Maryland Medicaid program that provides
long-term services and supports to eligible
adults in their own homes or assisted living
facilities rather than in a nursing home.
Covered services may include assisted living
(excluding room and board), case
management/supports planning, personal
assistance (help with bathing, dressing, etc.),
medical day care, home delivered meals, a
medical alarm, and environmental
accessibility adaptations (e.g., grab bars,
ramps).

Eligibility is based on income and assets of
the household. Monthly income may not
exceed 300% of SSI benefits. Countable
assets may not be more than $2,000 or
$2,500, depending on eligibility category.
Individuals must also meet a Nursing Facility
Level of Care.
The Harford County Health Department
strives to provide equal access to clients
who cannot communicate well in English.
Interpreting services are available free of
charge to all clients receiving our services.
The program currently has a significant
waiting list, known as the Waiver Services
Registry. To join the registry: Call Maryland
Access Point (MAP) at 1-844-627-5465 or
contact the Harford County MAP office.
Exceptions: Individuals currently residing in a
Medicaid-funded nursing home may be able
to apply directly to transition back to the
community through the Money Follows the
Person initiative.
