What is Care Coordination Plus?
Care Coordination Plus is a program that helps you receive the care you need. Our care coordinators work with the Comprehensive Care Center, UCMC/HMH, and other community agencies to connect you with community resources.
What can you expect from your Care Coordinator?
- Facilitate access to care by connecting individuals to health insurance coverage and clinical services.
- Assistance in navigating your insurance benefits.
- Clinical safety net services including linkage to various community resources.
- Partner with community agencies to help you coordinate services.
- Help you access community mental health services.
- A friendly professional to help you meet your healthcare needs.
- Partner with UCMC/HMH and the Comprehensive Care Center to help you meet your healthcare needs.
You can receive Care Coordination Plus if you:
- Are a Harford County Resident
- Have Medicaid, a Qualified Health Plan, Medicare, Commercial Insurance or are uninsured.
- Need help finding a doctor or navigating the system
