What is Care Coordination Plus?
Care Coordination Plus is a program that helps you receive the care you need. Our care coordinators work with the Comprehensive Care Center, UMMS, and other community agencies to connect you with community resources. Services provided in English and Spanish.
What can you expect from your Care Coordinator?
- Facilitate access to care by connecting individuals to health insurance coverage and clinical services.
- Assistance in navigating your insurance benefits.
- Clinical safety net services including linkage to various community resources.
- Partner with community agencies to help you coordinate services.
- Help you access community mental health services.
- A friendly professional to help you meet your healthcare needs.
- Partner with UMMS and the Comprehensive Care Center to help you meet your healthcare needs.
You can receive Care Coordination Plus if you:
- Are a Harford County Resident
- Have Medicaid, a Qualified Health Plan, Medicare, Commercial Insurance or are uninsured.
- Need help finding a doctor or navigating the system
