The Hospital Outreach Program (HOP) nurse assists individuals 18 years of age or older, referred by local hospitals or nursing facilities who are at risk for long-term care placement. The HOP nurse assists in locating continued-community resources, identifying barriers in the community, conducting follow-up visits to the home, coordinating with other community agencies including hospital discharge coordinators, and assisting the client with adherence to the hospital discharge plan and medication management.
