The Maryland Code of Regulations (COMAR) stipulates what conditions are required to be reported, who is required to report, how to report and specific reporting timelines.

Please review the following List of Conditions and What To Report, then complete the Maryland Confidential Morbidity Report form and fax it along with all pertinent lab results and visit notes to:
